What Is Achilles Tendinitis / Tendinopathy?

Achilles tendinitis, also known as Achilles tendinopathy, involves inflammation and degeneration of the Achilles tendon that connects the calf muscles to the heel bone. This tendon is essential for movements such as walking, running, jumping, and push-off actions. When repetitive strain exceeds the tendon’s healing capacity, small collagen fibers break down, leading to pain, stiffness, and reduced function.

Though “tendinitis” refers to acute inflammation, many chronic cases are actually tendinopathy — a degenerative disorder characterized by collagen deterioration, tendon thickening, and diminished structural integrity. Over time, tendons may lose elasticity and resilience, increasing the likelihood of partial tears or complete ruptures. Achilles tendinopathy frequently affects active individuals and athletes, though it can also affect non-athletes due to biomechanical problems or sudden increases in activity.

What Are the Causes?

Achilles tendinopathy arises from repeated mechanical stress combined with insufficient recovery. Sudden increases in training intensity, long runs, explosive jumping, and rapid accelerations place significant strain on the tendon. Contributing factors include tight calf muscles, restricted ankle mobility, inappropriate footwear, and training on hard surfaces, all of which amplify this load. Structural conditions such as flat feet, high arches, or abnormal gait mechanics influence force distribution and increase tendon stress. Moreover, age-related declines in blood flow and collagen elasticity reduce the tendon’s healing capacity, increasing injury risk for middle-aged and older athletes.

Symptoms

Symptoms typically develop gradually and worsen with activity. Pain is usually felt along the back of the heel or just above it, where the tendon is located. Morning stiffness is common and often improves with gentle movement. As degeneration progresses, discomfort may persist during walking, climbing stairs, or pushing off the ground. The tendon may appear thickened or feel abnormal to the touch. In later stages, weakness during push-off movements can occur. Without proper treatment, chronic tendon degeneration greatly increases the risk of partial or complete rupture.

How Many People Are Affected?

Achilles tendinopathy is a frequent overuse injury in the lower limb, commonly seen in runners, basketball and tennis players, and athletes engaged in repetitive explosive motions. Middle-aged recreational athletes, particularly those returning to activity after a period of inactivity, are especially vulnerable. Additionally, individuals with occupational strain or biomechanical issues can develop symptoms even without formal athletic activity.

Complications

Without proper management, Achilles tendinitis may progress to chronic tendinopathy, weakening the tendon. Continued degeneration increases the risk of partial tears or complete ruptures, potentially requiring surgery and prolonged rehabilitation. Chronic pain can also alter gait, putting extra stress on the knees, hips, and lower back over time. Early intervention greatly reduces the risk of lasting disability and functional loss.

Diagnosis

Diagnosis includes a comprehensive clinical assessment and physical examination, with emphasis on tendon tenderness, thickening, ankle range of motion, and calf muscle strength. Diagnostic ultrasound provides real-time imaging of tendon thickening, collagen fiber irregularities, neovascularization, and partial tears, and it is essential for guiding regenerative injections when needed. In more complex or advanced cases, MRI may be used to assess the extent of tendon degeneration or structural tears thoroughly.

Treatment Options

Treatment focuses on reducing mechanical overload, repairing tendon structure, and improving long-term function. Initial steps include modifying activities, engaging in targeted physical therapy with eccentric exercises, and correcting biomechanical problems. Supportive footwear and heel lifts can also help decrease tendon stress during the early recovery phase.

ESWT effectively treats chronic Achilles tendinopathy by delivering controlled acoustic pulses that stimulate the tendon and promote healing. Platelet-Rich Plasma (PRP)

PRP therapy may be considered if conservative rehab fails to produce lasting improvements.
